Prepackaged migration prompts

The migration toolkit stores ordered prompts under dynamics-migration-tool/prompts/. Your AI agent executes them in the sequence below and calls record-prompt-execution.sh after each applicable prompt so that executedPrompts[] in bootstrap.json stays current.
