Create a fill-in-the-blank question.

1. In the Create new list, click Fill in the Blank.
2. In the Text field, type a question.
3. In the Options field, type each correct answer on a separate line.
4. Click Save.

Create a multiple choice question.

1. In the Create new list, click Multiple Choice (single select).
2. In the Text field, type a question.
3. In the Style list, select the number of answer choices for the question. The default is five answer choices.
4. In the Options fields, type the answer choices.
5. In the Correct list, select the option beside the correct answer for the question.
6. In the Orientation column, select an option to display the answer choices.
7. Click Save.

Create a true or false question.

1. In the Create new list, click True/False.
2. In the Text field, type a question.
3. To change the answer choices from True and False, in the Options fields, type different answer choices.
4. In the Correct column, select the option beside the correct answer for the question.
5. In the Orientation column, select an option to display the answer choices.
6. Click Save.

Create a ratings scale question. This type of question is only available for a survey.

1. In the Create new list, click Ratings Scale.
2. In the Text field, type a question.
3. In the Style column, in the first list, select the style for the rating.
4. In the second list, select the number of rating choices for the question. The default styles are Numeric and 5 options. The available options for the style for the ratings are displayed in the Options column.
5. In the Orientation column, select an option to display the rating choices.
6. Click Save.
